PHA-F Membership is open for Federal Government Employees and general public (I-12/1, Islamabad), including all Pakistani citizens, Azad Kashmir and Gilgit Baltistan, and overseas Pakistanis of 18 years of age or above having valid NADRA CNIC are eligible for membership.

• Membership fee is Rs.5000/- (for Federal Government Employee) and Rs.25,000/- (for general public). (Non-refundable) and valid for only I-12/1, Project, Islamabad.
• Membership form along with demand draft shall be submitted in head office of PHA Foundation, Islamabad.
• Please type or print your particulars in the form in CAPITAL letters.
• All entries in the forms must be duly filled in English.
• Cutting / overwriting or ambiguous entries shall not be accepted.
• Latest photograph shall be affixed on form where specified duly attested by gazetted officer of Government of Pakistan.
• Attested copy of CNIC shall be enclosed as well as pasted on the form, duly attested by gazetted officer of Government of Pakistan.
• In case of overseas Pakistanis, attested copy of Passport shall be enclosed, duly attested by gazetted officer of Government of Pakistan.
• Signature on application should match with Computerized National Identity Card (CNIC).
• Demand Drafts of Rs.5000/- (for Federal Government Employee), Rs.25,000/- (for General Public) and Documentation charges of Rs.15,000/- (Non-refundable) in favor of “PHA Foundation, Islamabad”, should be submitted along with membership form, requisite Down payment.
